National Disability Insurance Scheme (NDIS)

Community Links Wellbeing is a registered provider with the National Disability Insurance Agency, and is third party verified by the Institute of Healthy Communities. We can assist people with disabilities who have funding approved for the following services:

  • Support Coordination
  • Individual Counselling
  • Behaviour Support
  • Therapeutic Supports
  • Early Childhood Supports
  • Assist-Life Stage
  • Transition
  • Speech Pathology
  • Occupational Therapy
  • Psychology
  • Exercise Physiology

We are here to give you a free helping hand to understand the NDIS, and get your supports in place. You do not have to take up our service after receiving information from us.

How Do We Work With You?

  • If you are confused and overwhelmed by the NDIS and need assistance to understand what it is all about then you can call us for information and support with no obligation
  • If you have your approved NDIS plan, you or your support coordinator can contact CLW to arrange a meeting
  • CLW will assess whether we can provide the services you are requiring
  • CLW will prepare a Service Agreement  for the services we will provide for you
  • You will be contacted by a staff member to book in the services we have agreed to provide for you
  • We will then bring your plan to life in line with your requirements. For example; you may have funding in your plan for Improved Daily Living. We will explain what Improved Daily Living is, how much you are funded for, what types of activities this includes and how many hours you have been funded for. We will then link you into the services to make this happen.
